Useful Parkinson's Passport:

The Parkinson’s Passport enables you to complete an information booklet about your medications and treatment and then carry it when you are out and about or travelling abroad. This passport has been created thanks to the active involvement of the EPDA membership, healthcare professionals and our industry partners.

Instructions for use

The new version of the Parkinson's Passport, developed in September 2018, includes information about:

• Medication dosage and timing
• Apomorphine
• Duodopa
• Deep brain stimulation
• Before you print out your Passport, you can personalise it by clicking onto the pages and completing the information that you wish to include. You can also print off additional medication sheets if required.

This document is for you to use as you need. You may want to take it to your doctor so he/she can help you fill in the details if you are unsure.
